Social Determinants of Health are the non-medical factors that influence health outcomes. These include the conditions in which people are born, grow, live, work, and age and the broader systems that shape daily life.
Access to safe housing, nutritious food, education, job opportunities, transportation, and supportive social networks all play a critical role in a person’s physical and mental well-being.
