Leading a strategic one-year initiative titled "Addressing Loneliness Among Older Adults Experiencing Serious Illness through Effective, Trusted Community Leaders," developed in collaboration with prominent national healthcare stakeholders. This project mobilizes trusted community connectors, Community Health Workers, and Community navigators to serve as "bridges of trust" for underserved populations, aiming to mitigate the adverse public health effects of social isolation. The role involves overseeing the identification and evaluation of these key community figures to validate the intervention’s effectiveness and secure sustainable support for future community care programs.
