On February, we officially launched the Connectedness Project / Compassionate Neighborhood, Homestead, a one-year initiative focused on addressing loneliness among older adults experiencing serious illness. This initiative is led by The Coalition to Transform Advanced Care (C-TAC) and generously supported by The Humana Foundation, and KCC is honored to serve as the community partner bringing this work to life in Homestead.
C-TAC has been advancing innovative, community-centered approaches to serious illness care across the country. Following the success of a previous Connectedness Project in Louisville, Kentucky, they recognized the need to expand this work to South Florida and chose to partner with Keralty Compassionate Communities (KCC) based on our experience building trust, connection, and care within communities.
While this is a one-year project, it is intentionally designed to serve as the starting point for KCC’s Compassionate Neighborhood in Homestead. This initiative allows us to kick off and strengthen a long-term, high-impact community effort, one that begins with older adults experiencing serious illness, but that we hope to continue expanding over the coming years to support people of all ages and needs across Homestead, guided by what the community itself identifies as priorities.
As part of this early phase, we are actively listening to and connecting with existing community resources and organizations that are already serving Homestead, including local health and social service providers such as Sanitas Medical Centers, Vida Medical Centers and Touching Miami With Love. These initial conversations help us better understand the local landscape and identify opportunities for collaboration as needs emerge and relationships continue to grow.
At the heart of this work is a simple but powerful idea: trusted community leaders matter. By strengthening the role of Community Connectors, Community Health Workers, and Community Navigators, those who listen, guide, and walk alongside older adults, we aim to bridge gaps in care, reduce isolation, and foster meaningful human connection where it is needed most. This project reflects our shared commitment to community-led solutions, dignity in care, and the belief that no one should feel alone while navigating serious illness.
